Job Title: Lead Case Manager– My Home Permanent Housing Program

Reports To: Director of Housing

FLSA:  Full-time/Salaried

This position is responsible for the coordination, planning and implementation of services for the Guest House permanent housing program. The Lead Case Manager is expected to be available on a 24-hour “on-call” basis for emergencies. The position reports to the Director of Housing of the Guest House.

Essential Duties and Responsibilities:

  • Provide direct supervision and management of case management team to include scheduling, educating, counseling, evaluation, placement and administrative functions.
  • Oversee and observe an assessment of the client to determine if client is eligible for the program.
  • Create, implement and review the clients service plan and programmatic policies and procedures.
  • Provide on-going communications with landlord, housing staff and service providers.
  • Act as an authorized proxy in the absence of the client.
  • Assure that all guest records and other records are documented and preserved in a correct fashion.
  • Responsible for expenditures, budgeting in compliance with HUD guidelines. Ensure that the program is administered in accordance with the contract.
  • Assure all relevant documentation relating to the operation of the program are maintained and updated as needed.
  • Ensure that all laws are complied with.
  • Other duties as assigned.
  • This position will also be responsible for Cream City Garden responsibilities as assigned.

Qualifications:

  • Knowledge and experience of utilizing community services and housing resources.
  • Ability to communicate clearly, both verbal and written. Ability to obtain necessary information for the housing eligibility interview.
  • Ability to write professional and technical documents.
  • Ability to plan, implement and follow through on projects relating to the position.
  • Supervisory experience required.
  • Ability to provide case management for eligible targeted population.
  • Skill and understanding in building management, landlord tenant relations and an understanding of housing contracts and leases.
  • Understanding of issues relating to homelessness and mental health.
  • Maintain a valid Wisconsin driver’s license, vehicle insurance, and reliable transportation in compliance with agency requirements.

Training and Experience:

  • Bachelors Degree in Social Work, Human Services or a related field required. Some education and experience may substitute for degree.
  • Excellent leadership and people skills.
  • Experience working with individuals who have experienced homelessness or who have/are living with mental illness.
  • Crisis Prevention and Intervention training.
  • Computer proficient – working knowledge of Microsoft programs.
  • First Aid training and CPR certification a plus.
