General Purpose

Managing immediate health needs of residents in Residential Treatment.  See clients in other programs as needed to help meet their immediate needs such as bridging prescriptions and consulting with their clinical team. Assigned nursing services as needed by various funding sources.

      Essential Functions of the Job

  • Supervise Med Tech to ensure cohesive care for Residential Clients especially related to ensuring all prescribed medications are available for clients
  • Responsible for providing direct client care to residential clients, effectively using the nursing process as the foundation for care delivery
  • Assist with admission decisions: helping determine health implications for possible admits into CBRF
  • Perform initial resident screenings including patient history, screen for communicable illnesses, and physical exams
  • Perform skilled nursing procedures: vital signs, blood pressure etc.
  • Assist with medication management decisions, prescribe medications as needed and work with medical director and outside providers to ensure health and safety of our residents
  • Provide leadership and direction within the clinical programming to support a team-based client-focused service model that promotes optimal health and well-being.
  • Work closely with pregnant and post-natal residents to help achieve healthy birth outcomes and mothers who feel safe and supported
  • Participate in development of policies and procedures that affect resident health: nutrition, exercise, smoking cessation and medications
  • Passionately supports a tobacco-free environment and helping people quit tobacco use
  • Open communication and strong teamwork with all disciplines to ensure resident receives quality healthcare
  • Consult with clinical team members in all programs and assist Meta House clients as needed within scope of practice
  • Demonstrate cultural sensitivity
  • Complete all required records within the EHR
  • Train and supervise your team in mandatory reporting policies and procedures

Marginal Functions of the Job 

  • Provide staff trainings
  • Other duties as may be periodically or permanently assigned

Qualifications and Requirements

  • Current licensure with Wisconsin’s Board of Nursing
  • Master’s in nursing with 5 plus years in Nursing.
  • Psychiatric Nurse Practitioner preferred
  • Have or be willing to obtain waiver to prescribe buprenorphine
  • Strong knowledge of substance use disorder and Mental Health
  • Demonstrated passion for working with at-risk populations
  • Excellent assessment and critical thinking skills
  • Experience supervising others

  • Basic computer skills – familiar with EHR medical charting system
  • Provide training/information to community partners, internal staff and donors as requested by the VP of Clinical Services

  • Participate in community resource meetings as assigned
  • Other duties as may be periodically or permanently assigned

Working Conditions and Physical Requirements

  • All Meta House staff are required to provide proof of vaccination for MMR, Varicella, COVID-19, and annual flu shot.
  • Adherence to a non-smoking work environment.

Able to:

  • Tolerate occasional hot or cold days.
  • Sit or stand for extended periods of time including alternating sitting or standing at will.
  • Lift and up to 25 pounds on a regular basis and 40 pounds on occasion.
  • Ascend and descend stairs using feet and legs.
  • Bend, stoop, twist, squat, and kneel to perform essential functions.
  • Reach overhead, and at or below shoulder level.
  • Read and enter text or data into a computer or other system.
  • Seize, hold, grasp, turn, or otherwise work with hands.
  • Touch, pick, punch or otherwise working with fingers.
  • Hear, understand, and distinguish speech and/or other sounds in order to communicate with clients and/or colleagues.
  • Express or exchange ideas by means of spoken word to impart oral information to clients or the public and convey detailed spoken instructions to others accurately, loudly, or quickly.
  • See clearly at approximately 20 inches or less including use of computers or tablet.
  • Visually identify clients and others for reasons including, but not limited to security, presence in the building, and medication dispensing.
  • Valid Wisconsin driver’s license required.
  • Ability to operate agency vehicle for work related duties.
  • Access to personal vehicle for work purposes.
  • Ability to use personal vehicle for work related duties.
  • Ability to use personal vehicle for work related duties including transportation of clients.
  • Auto insurance coverage with the following limits: 100k per person bodily injury/300k per accident bodily injury limit/100k property damage liability (required for employees using their personal vehicle for work or for employees transporting clients in their personal vehicle).
