General Purpose 

As a key member of the Residential Medical team, assist the Health and Wellness Practitioner (HWP) with all aspects of client care from admission through discharge. Partner closely with the Medication Technician and the residential care team to ensure continuity of physical and emotional care and safety for all residents, both internally and externally.

Essential Functions of the Job

  • Manage incoming phone calls, fax requests and emails to the medical team.
  • Obtain and document needed clinical information at admission (vitals, urine screen, pregnancy test, COVID screen, TB test).
  • Support continuity of medication orders, refills, documentation, and supply
    • Ensure all residents’ scripts are filled timely, received, and entered correctly into ECP, in collaboration with Med Tech
    • Check the Medical mailbox in each CBRF daily for overnight scripts or other correspondence that needs processing that day.
    • Assist HWP with medication/script transfers for discharging clients
    • Assist HWP with medication destruction, per policy
  • Support on-going medical appointment needs and support medical care follow-up
    • Schedule medical appointments with clients both internal and external, in collaboration with Peer Support Specialists and transportation team.
    • Work directly with clients on appointment reminders and life skills related to managing own appointments and follow up recommendations
    • Receive and process all incoming external documents (e.g. hospital records, lab results, clinic after-visit summaries, test results etc.); provide outreach to request these documents as needed.
    • Ensure all medical paperwork is scanned into chart and all medical charting is complete.
  • Prepare and maintain exam space and medical office in clean, organized, efficient order that is both safe and welcoming to clients.
  • Order supplies for medical suite.
  • Call for UA sample pick-ups with designated laboratory
  • Partner with and back-up the Medication Technician
    • Coordinate both cycle and non-cycle medication
    • Complete scheduled medication count and log
    • Pass medications to residents
    • Ensure accuracy, completeness, and organization of med cart
    • Inventory, order, and organize over-the-counter medications
    • Train staff on medication passing and answer general medical/medication procedural questions
  • Attend weekly Residential Staffing meeting and other meetings, as indicated

Marginal Functions of the Job

  • Provide input into Prior Authorization requests
  • Transporting clients to/from appointments
  • Running to pharmacy or completing other community errands
  • Participate in committees, workgroups, community meetings, or Meta House events.
  • Cross coverage of other MH roles on occasion, as requested and trained
  • Other duties, as assigned

Qualifications and Requirements

  • High School diploma or equivalent
  • Medical Assistant or CNA credentialing preferred
  • Prior experience in or sufficient knowledge of medical practice and care to assist in giving client care and supporting medical staff
  • Skilled in taking vital signs preferred
  • CBRF Certified, preferred
  • Basic knowledge of Substance Use and Mental Health Disorders
  • Driver’s license, safe driving record, willing to drive agency vehicles and insurable under Meta House’s insurance policy to drive agency vehicles preferred.
  • Auto insurance coverage with the following limits: 100k per person bodily injury/300k per accident bodily injury limit/100k property damage liability (required for employees using their personal vehicle for work or for employees transporting clients in their personal vehicle).

Working Conditions and Physical Requirements

  • All Meta House staff are required to provide proof of vaccination for MMR, Varicella, and annual flu shot.
  • Adherence to a tobacco-free work environment.

Able to:

  • Tolerate occasional hot or cold environments.
  • Sit or stand for extended periods of time including alternating sitting or standing at will.
  • Lift and up to 25 pounds on a regular basis and 40 pounds on occasion.
  • Ascend and descend stairs using feet and legs.
  • Bend, stoop, twist, squat, and kneel to perform essential functions.
  • Reach overhead, and at or below shoulder level.
  • Read and enter text or data into a computer or other system.
  • Seize, hold, grasp, turn, or otherwise work with hands.
  • Touch, pick, punch or otherwise working with fingers.
  • Hear, understand, and distinguish speech and/or other sounds in order to communicate with clients and/or colleagues.
  • Express or exchange ideas by means of spoken word to impart oral information to clients or the public and convey detailed spoken instructions to others accurately, loudly, or quickly.
  • See clearly at approximately 20 inches or less including use of computers or tablet.
  • Visually identify clients and others for reasons including, but not limited to security, presence in the building, and medication dispensing.
  • Valid Wisconsin driver’s license required.
  • Ability to operate agency vehicle for work related duties.
