Job Purpose and Reporting Structure

Under the direction of the Youth Shelter Program Manager, provides intensive street outreach and access to services for youth who are experiencing homelessness, have left home due to family conflict or crisis, or are living on the streets.

Essential Duties and Responsibilities

  1. Initiate street-based contacts and maintain street-based encounters with targeted youth that establish trust and allow for the provision of intensified survival aid and sexual abuse and exploitation prevention services.
  2. Provide outreach, direct referral, and transportation assistance connecting youth with emergency youth shelters. Offer intensive street outreach and ongoing supportive services to youth staying at Youth Shelters, coordinating services with the Youth Shelter team.
  3. Provide homeless verifications and collaborate with shelters and Coordinated Entry to connect youth with emergency shelter or safe and stable housing.
  4. Provide survival aid resources including basic needs (food, clothing), financial assistance, housing/ shelter, records and identification requests, and transportation assistance. Provide resource and referral information, including distribution of the project’s referral cards and assistance in accessing medical and mental health services. Provide trafficking and/or abuse prevention services, including advocacy, safety planning, crisis intervention, risk reduction resources, and assistance with exiting exploitive situations. Provide additional services including independent living skills development and assistance with school and employment concerns in collaboration with the Youth Shelter.
  5. Provide follow-up care to youth engaged through intensive street outreach services
  6. Coordinate with the RHY national communication system to provide resources to runaway and homeless youth
  7. Monitor, procure, and stock program equipment and resources in Outreach vehicle, and maintain Outreach vehicle as well as logging mileage, gas, and other vehicle related expenses.
  8. Complete monthly project reports, including service delivery statistics, client demographic information, client profiles, outcome achievement progress, and client risk assessment tools. Enter client data into HMIS system.
  9. Work effectively with the project’s collaborative partners to ensure the provision of street-based outreach services. Work effectively with the Youth Action Board and assist with project development, implementation, and evaluation.
  10. Maintain compliance with all applicable guidelines, policies, procedures, laws, and regulations.
  11. Provide project-related community outreach and promotional activities.
  12. Support the Youth Shelter donation process by helping to sort, store, donate, and otherwise maintain the donations area.
  13. Attend agency and community meetings and trainings.
  14. Other duties may be assigned.

Supervisory Responsibilities:

Supervision of AmeriCorps member(s) and/or student interns assigned to street outreach activities

Requirements:    

Requires education generally equivalent to a high school diploma.  Bachelor’s degree from a four-year college or university with emphasis in human services field preferred, but not required. Minimum of two years of experience working with youth preferred.

Additional information:
