JOB TITLE: Case Manager – Youth Homelessness Demonstration Program
SUPERVISOR: YHDP Program Manager
DESCRIPTION: Under the direction of the YHDP Program Manager, the YHDP case manager provides assistance to young adults seeking housing support by providing them with individual rapid rehousing rental assistance, basic needs resources, case management services, and access to specialized resources provided by various internal and external partners.
RESPONSIBILITIES:
- Assess housing barriers of individuals and families experiencing homelessness to determine their housing and services needs.
- Work cooperatively and effectively with the internal YHDP staff, Pathfinders YHDP staff, Coordinated Entry, Youth Action Board, and street outreach specialists in order to coordinate services and maximize youth access to necessary resources.
- Provide intensive and ongoing case management services to 17 participants at a time, focusing on finding and help participants maintain safe and secure housing.

- Assist participants in the development of strengths-based action plans that are individualized and client-centered and that support the youths/ development of positive connections, academic achievement, training and employment, safe and secure housing, and social and emotional well-being.
- Employ Housing First standards and principles to ensure low-barrier access to housing.
- Conduct weekly home visits in the participants’ apartments so that participants can work towards housing stability and toward goals they have set for themselves.
- Serve as an ongoing liaison between participants, Pathfinders landlord liaison, and other team members.
- Support participants both emotionally and physically, when they are moving into and out of apartments.
- Increase participants’ access to mainstream community resources within the Greater Milwaukee area. working effectively with the program’s collaborative partners to ensure successful service access.
- Maintain client records, keep accurate case notes, and complete necessary client reports as well as enter all data into HMIS.
- Participate in all relevant program and agency meetings, training, and development opportunities.
- Other duties as assigned.
POSITION REQUIREMENTS
BSW or equivalent Bachelor’s level social service degree or at least three years of equivalent experience; lived experience is also valued and will be considered when selecting candidates; strong communication skills (oral and written), and working knowledge of computer software; access to a car, valid Wisconsin driver’s license and proof (and maintenance) of auto insurance; ability to climb stairs, and some lifting. All candidates must possess knowledge of the Housing First model of service provision and must be willing to practice it.
SALARY $50,000 per year
