Balance provides a community-based Adult Day Program, Monday through Friday (8:15am-3:15pm) for adults with intellectual and developmental disabilities (IDD) located in Grafton.

Balance’s Mission is to partner with and support people with IDD to live fully, connect deeply, and thrive in community. Our Day Program supports this Mission by developing and providing activities to promote community involvement, social roles, and skill building.

The Adult Day Program focuses on health and wellness, living skills, community giveback, art and culture, and social development.

Program participants are supported in 1:1 care, 2:1 care (2 participants with 1 team member), or a small group of 4-5 individuals with one team member.

The Day Program operates approximately 75% of the time out in the surrounding communities.

A typical day consists of:

  • Arrival at Balance, engaging program participants with social time
  • Morning community-based, skill-building activity
  • Social lunch at Balance
  • Afternoon community-based, skill-building activity
  • Return to Balance, engaging program participants with socialization and skill building, prepare for heading home

Job Summary: The Direct Support Professional provides direct supervision and promotes engagement with participants in all of Balance Inc.’s services. The Direct Support Professional assists with activities of daily living including – personal care, companionship, leisure/ entertainment, meal preparation, medication and health reminders, dressing, grooming and personal hygiene, shopping, transportation, etc.

Key Responsibilities:

  • Work to help the people Balance supports develop the skills to thrive in community.
  • Work to help individuals with developmental disabilities develop their personal care, activities of daily living, communication and relationship skills.
  • Act as a role model and promote engagement in all activities.
  • Tell the story of the quality services you deliver by completing all required documentation associated with the program including but not limited to progress notes, data collection, incident reports, medication documentation, time sheets, etc.
  • Maintain a high degree of honesty and integrity at all times.
  • Provide transportation for residents using Balance’s mini and transit vans.
  • Acquire and maintain all required training to properly serve those we support.
  • Perform all aspects of job safely and using good judgment.
  • Maintain privacy and confidentiality of all individuals, families and employees, and comply with all Balance, state and federal privacy policies and laws.

Education: High School diploma preferred

Certifications and/or Licenses: Valid driver’s license required

Relevant Experience & Training:

  • Must be 18 years or older
  • Experience working with individuals with IDD preferred
  • Demonstrated ability to maintain confidentiality
  • Ability to work independently while exercising good judgment when solving problems

Working Conditions:

  • Work is performed in home and community-based settings. The Direct Support Professional will dress in casual attire that is presentable

Physical and Other Requirements:

  • Work requires sitting, standing, walking, crouching and driving; lifting up to 50lbs
  • Work requires physically supporting people with all aspects of life including – grooming/personal hygiene, eating, etc.
  • Complete a background disclosure form and meet all requirements to pass caregiver background checks
  • Pass initial drug screen and TB skin test and maintain a drug-free lifestyle
