About our Adult Day Program
Balance provides a community-based Adult Day Program, Monday through Friday (8:15am-3:15pm) for adults with intellectual and developmental disabilities (IDD).
Located in Grafton, Wisconsin, Balance’s Adult Day Program focuses on addressing the disparities that people with IDD face daily. Our goal is to improve each person’s quality of life by involving them in activities that take them out into the community at least once, and often two or more times per day.
Daily activities serve to enrich health and wellness, bolster creativity and socialization, and give participants opportunities to connect to their community through various outings and volunteer opportunities.
Program participants are supported in 1:1 care, 2:1 care (2 participants with 1 staff), or a small group of 4-5 individuals with one staff member.
Participants spend approximately 75% of their time out in the surrounding communities.
A typical day consists of:
- Arrival at Balance, social time
- Morning community outing
- Lunch at Balance
- Afternoon community outing
- Return to Balance, socialize with friends, and get ready to return home
Job Summary: The Direct Support Professional services as direct supervision for clients/participants in all of Balance Inc.’s services. The Direct Support Professional provides support, supervision, and assistance in performing activities of daily living including – personal care, companionship, leisure/ entertainment support, meal preparation and assistance, medication and health assistance and reminders, dressing, grooming and personal hygiene, shopping, transportation, and other assistance within the program.
Key Responsibilities:
- Work to help the people Balance supports develop the skills they need to be valued by other people.
- Work to help the people Balance supports create and maintain a living environment that encourages people to be a part of the community in which they live.
- Work to help individuals with developmental disabilities develop the personal care, activities of daily living, communication and relationship skills they need to be valued by other people.
- Work to support the people in the above areas where they cannot effectively support themselves.
- Act as a role model in all activities.
- Tell the story of the quality services you deliver by completing all documentation associate with the program including but not limited to progress notes, data collection, incident reports, medication documentation, time sheets, and other required documents.
- Maintain a high degree of honesty and integrity at all times.
- Transport people as needed and outlined in the individual’s plan using personal and/or Balance vehicles.
- Acquire and maintain all required and needed training to properly serve those we support.
- Perform all aspects of job safely and using good judgment.
- Maintain privacy and confidentiality of all individuals, families and employees and comply with all Balance, state and federal privacy laws.
Education: High School diploma preferred
Certifications and/or Licenses: Valid driver’s license required
Relevant Experience & Training:
- Must be 18 years or older
- Experience working with developmentally disabled individuals preferred
- Demonstrated ability to maintain confidentiality
- Ability to work independently while exercising good judgment when solving problems
Working Conditions:
- Work is performed in home and community based settings. The Direct Support Professional will dress in casual attire that is presentable
Physical Requirements:
- Work requires sitting, standing, walking, crouching and driving; lifting up to 50lbs
- Complete a background disclosure form and meet all requirements to pass criminal background checks
- Pass initial drug screen and maintain a drug-free lifestyle
