Job Summary: The Health & Wellness Advocate promotes the Balance mission by supporting people with Intellectual and Developmental Disabilities (IDD) to schedule, attend, communicate during, and follow up after routine and specialty healthcare appointments. The Advocate also develops and leads wellness promotion initiatives for all Balance team members and service users.
Key Responsibilities:
Healthcare Appointments
- Responsible for scheduling, assist with scheduling, and/or helping people with IDD develop skills to schedule routine and specialty healthcare appointments
- Ensures routine and specialty appointments are schedule as required by applicable federal, state, and funder regulations, and medical orders
- Promotes appointment attendance by collaborating with each person to support their unique needs
- Advocate, assist with self-advocacy, and help people with IDD develop self-advocacy skills for healthcare appointments
- Collect, document, and communicate or assist to collect, document, and communicate appointment details, medical orders, and follow up recommendations
- Supports the Director of Health Services with projects related to health care support including electronic health record (EHR) documentation and upkeep or other tasks as assigned
Wellness Initiatives
- Seeks resources and leads the implementation of wellness promoting initiatives for everyone at Balance
- Motivates, encourages, and models active engagement in wellness initiatives
- Builds community engagement through wellness initiatives
- Use resources and activities to increase wellness knowledge
- Maintains instructor certification & provides training to team members for CPR/First Aid and other related topics
Education:
High School degree or equivalency required, college degree and/or healthcare/wellness training is a plus
Certifications and/or Licenses:
- Valid driver’s license
- CPR/First Aid certification or ability to obtain
Requirements:
- Auto insurance
- Excellent driving record
- Start and end time flexibility
Relevant Experience & Training:
- Minimum of 2 years of experience with people who have IDD
- Ability to self-direct, manage time effectively with frequent interruptions
- Must be adaptable in times of unpredictability and exercise sound judgement
- Must have strong interpersonal, written and verbal communication skills, and ability to maintain professionalism and confidentiality
- Ability to develop, monitor and maintain positive and supportive relationships with a diverse group of team members, program participants, healthcare professionals, other external stakeholders
- Must have strong organizational skills
- Must have basic computer skills (MS Office suite) and ability to become a proficient user of Electronic Health Record and other industry related software
Working Conditions:
- Work is performed in a professional, creative, open office environment with business casual attire.
- Position is eligible for some remote working based on supervisor deaccession and business needs.
- Hours of work will generally be during regular business hours.
- Travel – frequent travel between sites and to healthcare appointments in the surrounding area
Physical and Other Requirements:
- Work requires sitting, standing, walking, crouching and driving
- Must frequently lift and/or move 10 to 25 pounds and occasionally lift and/or move up to 50 pounds
- Complete a background disclosure form and meet all requirements to pass criminal background checks
- Pass initial and periodic drug screenings and maintain a drug-free lifestyle
