Job Title: SOAR Benefits Specialist

Reports To: Director of Clinical Services

FLSA Status: Full-time/Salaried

This position is an ancillary service of the Comprehensive Community Services (CCS) Program at the Guest House of Milwaukee. The SOAR (SSDI/SSI Outreach, Access and Recovery) Specialist provides outreach and support to adults experiencing homelessness or at risk of homelessness that have mental illness and/or a co-occurring substance use disorder or other medical impairments apply for Social Security disability benefits.  The SOAR Specialist also works collaboratively with representatives from the Social Security Administration, Disability Determination Bureau, and other local providers to ensure quality and comprehensive disability benefits application.  With these benefits, it is our goal that our consumers will have resources to attain a higher quality of life.

Essential Duties and Responsibilities:

  • Assist consumers with the application for SSI/SSDI utilizing the SOAR principles and techniques.
  • Conduct bio-psycho-social assessments with consumers using a trauma informed approach.
  • Collaborate with the consumers’ recovery teams to collect collateral information and evidence to be used in the Medical Summary Report.
  • Gather medical records and other information to complete SSI/SSDI applications.
  • Coordinate and accompany clients to appointments with medical providers and the Social Security Administration.
  • Complete SOAR training with in the first 30 days of employment.
  • Track and report information on interventions, encounters and progress of application, completing record keeping in a timely manner.
  • Submit outcome data as requested by the Program Administrator.
  • Achieve a minimum of 70% approval rating.
  • Participate in local SOAR meetings as required.

Qualifications:

  • Experience working with individuals who are homeless or at risk of homelessness.
  • Experience working with individuals diagnosed with mental illness and or substance use disorders.
  • Knowledge of psychiatric and medical terminology.
  • Ability to prioritize tasks and meet frequent deadlines.
  • Excellent written and verbal skills, particularly compelling and comprehensive written communication skills.
  • Experience with completing bio-psycho-social assessments.
  • Ability to work with other agencies throughout the community in a collaborative manner.
  • Familiarity with federal and state public benefits programs and/or SOAR a plus.
  • Ability to work independently with initiative and commitment.
  • Maintain a valid Wisconsin driver’s license, vehicle insurance, and reliable transportation in compliance with agency requirements.

Training and Education:

  • A Master’s degree in Social Work or Counseling preferred or Bachelor’s degree in Social Work with at least two years’ experience working with individuals diagnosed with a severe and persistent mental illness or co-occurring disorders.
  • Training in trauma informed care, motivational interviewing and person-centered care.
  • Knowledge of federal SSI/SSDI application process.
