Job Title: Housing and Rehabilitation Specialist

Reports To: Director of Clinical Services

FLSA Status: Full-time/Salaried

This position is an ancillary service of the Comprehensive Community Services (CCS) Program at the Guest House of Milwaukee. The Housing and Rehabilitation Specialist provides client-driven services that aid consumers in addressing their socioeconomic needs including assistance with obtaining permanent housing, education and employment as well as modeling and assisting with the development and/or enhancement of individual life skills such as assistance with household chores, time management, scheduling and maintaining appointments, money management as well as social and recreational activities. This position requires attention and responsiveness to the consumer’s changing needs, conditions and goals during their participation in the CCS program. The Housing and Rehabilitation Specialist position requires close collaboration with consumer’s CCS Care Coordinator, therapist and other essential care providers to support the consumer in their recovery as they strive to progress towards their life vision and goals.

Essential Duties and Responsibilities:

  • Meet with consumer in the home or community as authorized on consumer’s Individual Recovery Plan
  • Oversee housing issues including advocating and/or mediating landlord/tenant issues
  • Assist clients with the development of daily living skills that include but are not limited to the upkeep of their apartment, budgeting and overall general money management
  • Assist consumers in locating housing, medical and mental health assistance,  medication management, developing informal social support systems, accessing AODA services and supports, assisting with developing/improving activities of daily living skills (ADLs) as identified in the consumer’s Recovery Plan of Care
  • Work collaboratively with the consumer’s recovery team to monitor services and consumer’s progress towards goals
  • Participate in mandatory CCS Recovery Team Meetings
  • May occasionally provide transportation for consumers
  • Collaborate with case managers for various housing programs such as Homelinc, Housing First, Keys to Independence
  • Participate in meetings with other community partners
  • Maintain confidentiality
  • Document daily activity and consumer progress in the form of case notes
  • Meet programmatic productivity goals
  • Participates in agency quality improvement projects when requested
  • Other duties as assigned


  • Experience working with individuals diagnosed with mental illness and or substance use disorders using person-centered and trauma informed approaches
  • Able to work flexible hours, including evening, weekends, depending on needs of the consumer
  • Understanding of the requirements related to HUD, City/County/State administered housing regulations and other available housing assistance programs.
  • Working knowledge and understanding of money management including budgeting
  • Ability to prioritize tasks and meet deadlines
  • Strong interpersonal skills and ability to communicate clearly and effectively
  • Ability to work with other agencies throughout the community in a collaborative manner
  • Excellent critical thinking; able to use logic and reasoning to solve problems
  • Maintain a valid driver’s license, vehicle insurance and reliable transportation in compliance with agency requirements

Training and Education:

  • Minimum Associates Degree in a human services related field, along with 2+ years of relevant work experience, Bachelor’s Degree in Social Work, Human Services or a related field required. Master’s Degree preferred
  • Computer proficient – working knowledge of Microsoft programs
  • First Aid training and CPR certification a plus
