About the Role

The Case Manager for Prairie Apartments works with people with disabilities to assist them in maintaining their supported permanent housing placement. This position requires flexibility and is responsible for creating an atmosphere of independence, yet belonging to those residents who live within Prairie Apartments. This position is responsible for the coordination of activities for both the building community and for individuals to participate in the larger Milwaukee community.

Essential Duties and Responsibilities:

  • Initiates daily contact with each of the 24 residents at Prairie Apartments, conducting apartment visits at least twice per month.
  • Assists clients with daily living skills that include, but are not limited to, upkeep of their apartments, budgeting and money management.
  • Works in collaboration with Property Manager and any external service providers to ensure the appropriate services to residents are delivered.
  • Maintains tenant charts to ensure compliance with relevant county, state, and federal requirements, as well as with agency policy and practices.
  • Ensures all relevant documentation relating to the program is maintained and updated as needed.
  • Provides on-going communications with external agencies in relation to program goals and objectives.
  • Creates partnerships with outside entities to better serve the residents and schedules various activities for the tenants to participate in throughout the month and year.
  • Ensures tenants are following the rules of the program and staying in compliance with those rules.
  • Creates and maintains Annual Program Report tracking forms.
  • Maintains quarterly summaries for each of the residents.
  • Other duties as assigned by the Director of Community Housing Programs.

Qualifications:

  • Ability to communicate clearly, both written and oral.
  • Creative spirit and flexibility in schedule.
  • Previous experience in case management or related social services field.
  • Strong knowledge and understanding of mental health issues and addictions.

Training and Experience:

  • Bachelor’s degree in Social work, Psychology, Human Services, or related field preferred.
  • Knowledge of computer software including Microsoft Office including Access.
  • Commitment to the mission of the program.
