Your Role: 

As a Care Coordinator, you’ll be responsible for coordinating services for youth with mental health and substance use needs and supporting their families. You’ll serve as their advocate, connecting them with essential community resources and ensuring their individualized care plan is met by way of a strength-based, solution-focused, trauma-informed, and culturally humble approach.

JOB LOCATION: Social Services – Youth Programs

PRIMARY FUNCTION: Responsible for coordination of services for individuals enrolled in the CORE/CCS (Early Intervention) Program.

PRIMARY DUTIES:

  1. Facilitate all aspects of services for consumers (transport participants as needed, assemble teams, assess needs and strengths, develop an individualized Plan of Care (POC), complete functional screens, advocate, provide or secure services, etc.). E
  2. Serve as a liaison between the child/family and service providers and assure acquisition of services. E
  3. In collaboration with the team, monitor and negotiate provision of services and agreements, modify the POC and authorizations when new services are added and/or existing services discontinued, etc. E
  4. Follow and ensure all legal, organizational and contractual requirements, laws and policies. E
  5. Prepare, complete, distribute, and/or submit required items by due dates. E
  6. Attend meetings and professional development activities, as requested. E
  7. Establish and maintain good communication, collaboration, and cooperation with all stakeholders. E
  8. Follow all duties of a mandated reporter. E
  9. Perform other related duties, as assigned. E
  10. Be a team player. E

Requirements

WORKING CONDITIONS:

Work is generally performed in field and office environments, including travel. Local travel required; some state-wide travel as needed. Flexible hours required; as dictated by program needs.

REPORTS TO: Clinician/Therapist/Supervisor – CORE/CCS-Youth

MINIMUM QUALIFICATIONS REQUIRED:

  1. Bachelor’s Degree (Master’s Degree preferred) from an accredited school in Social Work or related field (i.e.; psychology, sociology, counseling).
  2. One (1) year of continued experience providing mental health services.
  3. Able to work and communicate frankly and effectively with adults, families, and all team members to implement problem solving strategies, understand issues, and to provide crisis services in an ethical, respectful, responsible manner.
  4. Able to demonstrate positive cultural intelligence with diverse groups and individuals.
  5. Excellent critical thinking, organizational skills, and able to manage multiple priorities.
  6. Excellent communication, verbal and written, interpersonal skills, and ability to work with others.
  7. Knowledge and experience with experience in using Microsoft Office suite.
  8. Transportation, valid Wisconsin Driver’s license, state minimum auto insurance, and must meet La Causa, Inc. driving standards.
  9. Must be able to successfully pass criminal history background checks.
  10. Must be able to work flexible hours based on program needs.
