PRIMARY FUNCTION: Responsible for providing assistance to individuals, as referred by the WIser Choice program, to form a Recovery Support Team in order for the client to successfully achieve their goals of the Single Coordinated Care Plan while providing continued assistance and monitoring of the client until these services are no longer necessary and/or appropriate.

PRIMARY DUTIES:

  1. Facilitate all aspects of initial contact services to include, but not limited to: locate and make initial contact with referred client, develop a preliminary Single Coordinated Care Plan (SCCP) with the client (as applicable, transition SCCP plan may be required prior to the client’s release), assist the client to develop a Recover Support Team, etc. E
  2. Facilitate all aspects of continuing client support services to include, but not limited to: provide case management services (transportation, appointments, independent housing search, legal advocacy, benefits, court hearings, etc.), provide 24-hour, 7-day-a-week crisis access, refer individuals with less intensive needs to applicable services, maintain contact with client and recovery team, submit preliminary and subsequent versions of the SCCP, assist the client with choosing and accessing services, etc. E
  3. Facilitate all aspects of monitoring, reporting, and compliance to include, but not limited to: update client information, monitor, update, and respond accordingly to the SCCP, monitor service authorizations to ensure they are current, request extensions of additional service authorizations, monitor and document the provision of services by agencies, complete the required outcome data collection and client satisfaction instruments, ensure compliance with program completion measurement protocols, develop and document a plan that identifies sources to pay for services, negotiate the allocation of funding among various system representatives, etc. E
  4. Follow legal, organizational, and contractual requirements, laws and policies (i.e.; County Contract). E
  5. Prepare, complete, and submit required items by due dates and required timelines. E
  6. Establish and maintain good communication, collaboration, and cooperation with all stakeholders. NE
  7. Implement and/or practice the duties and responsibilities of a mandated reporter of suspected child abuse under current policies and procedures, as required. E
  8. Attend all meetings and professional development activities, as directed. NE
  9. Perform other duties as assigned. NE

The employee is regularly required to drive, stand, sit, reach, stoop, bend, and walk. Employee is frequently required to see, talk, or hear. Finger dexterity is also required. Infrequent lifting, including files, will be encountered.

WORKING CONDITIONS:

Work is generally performed in field and office environments; including travel. Local travel required; some state-wide travel as needed. Flexible hours required as dictated by program needs.

MINIMUM QUALIFICATIONS REQUIRED:

  1. Bachelor Degree in Social Work, Psychology, Nursing, or related field required.
  2. One (1) year of experience in human services, preferably case management, required.
  3. Bilingual (Spanish and English), oral and written, preferred.
  4. Able to work and communicate frankly and effectively with applicable stakeholders to implement problem solving strategies, understand needs, and to provide services in an ethical, respectful, responsible manner.
  5. Able to demonstrate cultural intelligence with diverse groups and ability to work with individuals from diverse economic backgrounds in a respectful manner.
  6. Excellent communication and documentation skills, written and verbal, appropriate to target audiences across multiple functional areas, categories, levels, linguistic, and economic backgrounds; both in a team and individual environment.
  7. Excellent critical thinking; able to use logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems.
  8. Highly developed organizational skills and able to manage multiple priorities.
  9. Knowledge and experience with experience in using Microsoft Office suite.
  10. Able to work flexible hours, including evenings and weekends depending on needs of the program needs.
  11. Must successfully complete and pass all background checks.
  12. Transportation, valid Wisconsin Driver’s license, state minimum auto insurance, and must meet La Causa, Inc. driving standards.

Salary Description

$38,889.61 – $48,228.90 Based on Experience
