In the role of Social Worker II you will provide clinic-based medical social work consultation services to the LGBTQ+ community of patients receiving their care in the Inclusion Health Clinic at Froedtert Hospital. Medical social work services may include care coordination, benefits access and navigation, health literacy support, health system navigation and advocacy, referral, and community care coordination. To participate as a member of a multidisciplinary care team providing support to members across the LGBTQ+ community.

Primary Functions

  • Responsible for triaging and addressing urgent social service needs of patients receiving care in the Inclusion Health Clinic.
  • Assist patients enrolling in, navigating, and understanding health insurance benefits.
  • Work collaboratively with clinic providers serving as a consultative content expert addressing acute social service questions.
  • Provide social work services (non-case management needs) to individuals seen in the Froedtert Inclusion Health Clinic with a diverse range of socially and medically unique needs.
  • Respond to the care navigation questions of patient’s family members and support systems.
  • Triage of emergent situations, phone conversations and the ability to negotiate healthcare and social service systems.
  • Interface with clinical staff and support social service peers with immediate clinic questions.
  • Provide social service referrals and ensure successful linkage to medical and social service resources across the community.
  • Deliver health-related education and health literacy development.
  • Support the development and implementation of social work workflows across the clinic.
  • Implement social work assessment and care plan tools for patients referred for case management interventions.
  • Complete documentation requirements for all services provided and participate in clinical supervision/consultation in compliance with all state licensing requirements.
  • Serves as a team member on a multidisciplinary medical care team, communicating with team members to ensure comprehensive and quality wrap-around support.

Knowledge – Skills – Abilities

Knowledge of medical systems, health care setting and LGBTQ+ specific health care needs. Understanding of insurance benefits available for patients across social service systems. Ability to provide mental health screening and appropriate referral, as well as crisis triage and management. Ability to function as an interdisciplinary team member. Sociocultural sensitivity. Ability to respond to clinical and administrative emergencies. Education and/or experience in outpatient interventions in community social service and mental health settings.

Knowledge of biology, clinical judgment, medicine, cultural awareness, and consulting.

Medical documentation, interpersonal, practice-based learning and improvement, empathy, and decision making skills.

Specifications

Minimum Qualifications:

  • Master’s degree in Social Work, Counseling or other Mental Health related field.
  • Experience with coordination and care navigation health care systems.
  • One to three years’ social work experience.
  • Certified Social Worker or Advanced Practice Social Worker (State of Wisconsin).

Preferred Qualifications:

  •  Experience in advocating for the LGBTQ+ community strongly preferred.
  •  Certification in Motivational Interviewing strongly desired.
