Job Purpose and Reporting Structure:

Under the supervision of the Director of Comprehensive Community Services, the Comprehensive Community Services (CCS) Housing Specialist provides guidance and support related to housing to youth ages 23 and under with a mental health diagnosis or substance abuse diagnosis living in Milwaukee County. The CCS Housing Specialist will support the wellness plan designed to provide the highest level of independent functioning and quality of life possible and desired by the youth. The CCS Housing Specialist will work in alliance with the youth’s care coordinator, working toward helping the youth build the skills needed to locate, secure and maintain stable housing.

Essential Duties:

  1. Assist youth ages 23 and under with a mental health diagnosis or substance abuse diagnosis navigate the housing market by locating, applying and securing housing within Milwaukee County.
  2. Provide education to youth in relation to coordinator entry’s system of care and Milwaukee County housing vouchers.
  3. Teach youth how to maintain stable housing, improve problem solving skills, conflict resolution skills, and other specific needs related to the youths’ housing.
  4. Advocate for youth’s housing needs with landlords, community members and coordinated entry’s system of care.
  5. Facilitate positive activities that encourage positive social interactions with other adults and peers in all areas of the youths’ lives, including community, home, school, and recreation.
  6. Attend the youth’s scheduled team meetings and treatment plan meetings to advocate for additional services or needs that would benefit overall care and success of the youth.
  7. Generate and maintain accurate documentation as required by established standards by Pathfinders, Wraparound Milwaukee, and Medicaid.
    • Maintain youth records and complete all necessary reports of activities, inputs, outputs, and outcomes. Paperwork must be completed in a strength-based manner.
    • Assure accurate and timely documentation and billing in the Wraparound Synthesis database.
  8. Increase youths’ access to mainstream community resources available through the Drop-In Center and Pathfinders Youth Shelter. Work effectively with program’s collaborative partners to ensure successful service access.
  9. Participate and represent Pathfinders in all meetings for the CCS program within Pathfinders and with Wraparound Milwaukee.
  10. Participate in all program and agency meetings, trainings, and development opportunities.
  11. Adhere to all relevant Pathfinders Milwaukee and Wraparound Milwaukee Policy and Procedures.
  12. Other duties may be assigned.


  • Requires Bachelor’s Degree in Social Work or human services field.
  • Requires knowledge and belief in Comprehensive Community Services philosophy and strategies.
  • Experience working with a diverse population of adolescents and young adults within a nonprofit setting is required.
  • Must be willing to work a flexible schedule of morning, afternoon, and evening hours.
  • Must have excellent communication and interpersonal skills, and be able to maintain privileged client information.
  • Must have valid driver’s license, reliable vehicle, and proof of auto insurance.

Additional information:
