We seek Care Coordinators to work within our youth programs. Generally speaking, the Care Coordinators are the primary contact person/facilitator of the care team. Care Coordinators usually support about 8 to 12 individuals and their families at any one time, maintaining a minimum of weekly contact by phone and/or in-person visits. This position is responsible for the delivery of service and support for the people we serve. Care Coordinators meet with individuals, families, service providers, and other team supports that assist the individual through their specific program to efficiently and effectively address their needs and work toward sustainability.
Care Coordinators monitor the provision and quality of services provided to the individual and are the liaisons when new services/resources are sought or developed. Care Coordinators coordinate and facilitate team meetings to develop and modify care plans. The position is also responsible for seeking out community resources and ensuring the individualized plan of care is up to date.
Care Coordinators schedule appointments and provide or make arrangements for transportation. If appropriate, Care Coordinators attend Court proceedings and other appointments in the community to provide support and advocate for the individual we are serving. Care Coordinators are expected to meet and work with the individuals we support in their homes and communities.

All Care Coordinator positions require a bachelor’s degree in a related field. Some require additional training or experience. Because the position requires local travel within the community to agencies and residences, additional qualifications for this position include a valid driver’s license, automobile, and automobile insurance. All employees must meet pre-employment requirements including reference checks, criminal and caregiver background checks, drug screen, and driver’s license check to safeguard the people/e we serve.
