The Part-Time INSIGHTS Case Manager provides comprehensive case management services to youth and young adults participating in the INSIGHTS Transitional and Supportive Housing Program. Working from a Housing First and trauma-informed approach, the Case Manager assists participants in achieving housing stability, increasing self-sufficiency, and connecting to community resources that support their long-term success.
This position requires regular community-based services, collaboration with community partners, and ongoing engagement with participants to ensure individualized service planning and goal achievement.
Essential Job Responsibilities
- Maintain an active caseload of at least eight (8) program participants.
- Meet with each assigned client a minimum of once per week, with additional contacts as needed based on client needs.
- Complete comprehensive assessments and develop individualized service plans in collaboration with participants.
- Assist participants in identifying and achieving personal goals related to housing stability, education, employment, health care, financial stability, and overall well-being.
- Connect participants with community resources, including:
- Housing programs
- Mental health services
- Medical care
- Substance use treatment
- Employment services
- Educational opportunities
- Financial assistance
- Food and basic needs resources
- Assist participants with housing search activities, landlord communication, lease compliance, and maintaining stable housing.
- Conduct home visits and community-based meetings as appropriate.
- Participate in Coordinated Entry meetings and collaborate with community partners to support housing placement opportunities.
- Advocate for participants when navigating community systems and accessing services.
- Maintain accurate, complete, and timely case documentation in accordance with agency, funder, and regulatory requirements.
- Participate in case consultations, supervision, staff meetings, and required trainings.
- Maintain participant confidentiality in accordance with HIPAA and agency policies.
- Provide crisis intervention, safety planning, and referrals as appropriate.
- Collaborate closely with the INSIGHTS Program Manager and multidisciplinary team to coordinate services and support positive participant outcomes.
- Perform other duties as assigned.
Required Qualifications
- Experience working with vulnerable youth, young adults, or individuals experiencing homelessness.
- Knowledge of trauma-informed care, strengths-based case management, and Housing First principles.
- Excellent communication, organizational, and time management skills.
- Ability to work independently while maintaining strong collaboration with team members.
- Ability to establish professional relationships with participants from diverse backgrounds.
- Strong problem-solving, advocacy, and crisis intervention skills.
- Proficiency with Microsoft Office and electronic documentation systems.
Preferred Qualifications
- Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, or a related field preferred.
- Relevant professional experience may be considered in lieu of a bachelors degree.
- Previous experience providing housing-focused case management is strongly preferred.
- Knowledge of Milwaukee County housing resources, Coordinated Entry, and homeless services systems is preferred.
Compensation: $18.00–$20.00 per hour, based on education and experience
