Outreach Case Manager

Walker’s Point Youth & Family Center (WPYFC), in partnership with Inner Beauty Center (IBC), is seeking a compassionate, motivated, and trauma-informed Outreach Case Manager to provide outreach, engagement, and intensive case management services to youth and young adults between the ages of 11 and 25 who are currently experiencing or are at risk of commercial sexual exploitation, sex trafficking, or involvement in the sex trade.

This position works collaboratively alongside Inner Beauty Center staff during street outreach efforts, community engagement activities, and within the Inner Beauty Center Drop-In Center. The Outreach Case Manager will build trusting relationships with participants through a harm reduction and client-centered approach while connecting individuals to critical services including housing, healthcare, mental health treatment, substance use treatment, education, employment, legal assistance, and other community resources.

The ideal candidate understands that healing occurs through relationship building, consistency, and meeting individuals where they are without judgment.

Essential Duties and ResponsibilitiesOutreach & Engagement

• Participate in scheduled street outreach efforts alongside Inner Beauty Center staff.
• Engage youth and young adults in locations where they naturally congregate using trauma-informed and harm reduction practices.
• Build trusting relationships with individuals experiencing exploitation, homelessness, substance use, and mental health challenges.
• Provide crisis intervention, safety planning, and emotional support during outreach contacts.
• Distribute basic needs of supplies including hygiene items, food, clothing, transportation assistance, and other outreach materials.
• Assist in the operation and programming of the Inner Beauty Center Drop-In Center.
• Maintain professional boundaries while developing meaningful therapeutic relationships.

Case Management

• Maintain an active caseload of up to 18 clients.
• Complete comprehensive assessments and individualized service plans.
• Meet clients regularly based upon their individualized goals and needs.
• Assist participants in obtaining:
  • Safe and stable housing
  • Emergency shelter
  • Mental health services
  • Substance use treatment
  • Medical and dental care
  • Employment opportunities
  • Educational and vocational programs
  • Identification documents
  • Public benefits
  • Legal advocacy
  • Transportation resources
• Provide advocacy with landlords, employers, schools, healthcare providers, and community agencies.
• Support participants in setting and achieving individualized goals.
• Accompany clients to appointments when appropriate.
• Conduct home visits, community visits, and outreach contacts as needed.

Documentation & Compliance

• Maintain accurate, timely, and confidential client records.
• Complete required assessments, case notes, referrals, and program documentation within agency timelines.
• Collect outcome data and maintain documentation required by funding sources.
• Participate in coordinated care meetings and interdisciplinary team meetings.
• Ensure compliance with agency policies, confidentiality standards, and applicable regulations.

Collaboration

• Work closely with Walker’s Point Youth & Family Center and Inner Beauty Center staff to coordinate services.
• Build and maintain positive relationships with community providers.
• Participate in case consultations and multidisciplinary team meetings.
• Assist in identifying service gaps and developing creative solutions for participants.
• Represent the agency professionally within the community.

• Participate in agency meetings, supervision, and required trainings.

QualificationsRequired

• Bachelor’s Degree in Social Work, Human Services, Psychology, Criminal Justice, or a related field preferred.
• Equivalent combination of education and significant related experience will be considered in lieu of a degree.
• Minimum of 2 years of experience working with one or more of the following populations:
  • Individuals experiencing commercial sexual exploitation or sex trafficking
  • Individuals involved in the sex trade
  • Youth experiencing homelessness
  • Individuals living with mental health challenges
  • Individuals experiencing substance use disorders
• Strong understanding of trauma-informed care and harm reduction principles.
• Knowledge of Milwaukee-area community resources.
• Excellent communication, organization, and documentation skills.
• Ability to establish rapport with highly vulnerable populations.
• Ability to work independently while collaborating effectively with multidisciplinary teams.
• Ability to maintain confidentiality and exercise sound professional judgment.

Preferred

• Wisconsin Social Work certification or eligibility for certification.
• Experience with street outreach programming.
• Experience using HMIS or other electronic case management systems.
• Bilingual abilities are highly valued.
• Experience working with youth and young adults ages 11–25.

Physical Requirements

• Ability to lift up to 25 pounds.
• Ability to walk for extended periods during outreach activities.
• Ability to navigate stairs and uneven terrain.
• Ability to travel throughout Milwaukee County.
• Ability to work outdoors in varying weather conditions during outreach activities.

Requirements

• Valid Wisconsin Driver’s License.
• Reliable personal vehicle.
• Current automobile insurance meeting state minimum requirements.
• Ability to pass required background checks.
• Current CPR/First Aid certification (or ability to obtain upon hire).
